How to read these. These are the ratios we look for when a practice is running well at that size. They are targets, not rules, and a practice can be healthy while sitting outside one of them for a good reason. What matters is the direction of travel. A solo who is spending 22% on overhead against a target of 33% is not failing, they are running lean. A mid-size group paying 68% to clinicians against a 45 to 60 band has a real math problem, and it will show up as an owner who works every weekend and takes home less than the staff.
